The Diocese of Raleigh provides a Life Insurance benefit for ALL employees working 30 hours or more per week. The value of each employee's life insurance is equal to 2 x the employee's salary rounded up to the next thousand.* The Diocese of Raleigh does NOT offer supplemental life insurance. Dependents of eligible employees can not purchase life insurance through the Diocese of Raleigh.

Enrollment

Enrollment in the Life Insurance plan is done at the time of enrollment for medical/dental benefits on the CBEBT Request for Group Coverage/Enrollment Form.

If an eligible employee waives coverage for medical and/or dental benefits, they will still be enrolled in the Diocese of Raleigh Life Insurance plan.

A beneficiary (or beneficiaries) must be designated at the time of enrollment.

Salary Increases/Change in Beneficiary

Increases in an employee's salary must be reported to CBEBT using the Address/Salary Change Form in order for the value of an employee's life insurance to be correct. CBEBT will report the increase to Reliance Standard Life Insurance Company.

Life insurance benefits are reduced after age 65 for eligible employees still employed at that time.

Changes in a beneficiary (or beneficiaries) require the Designation of Beneficiary form to be completed. All beneficiary forms must be sent to the Benefits Office at the Catholic Center, attention Nancy von Gunten.

Filing a Life Insurance Claim

All claims for life insurance benefits must be handled through the Benefits Office at the Catholic Center. Please notify Nancy von Gunten, Benefits Administrator at (919)821-9775 if an eligible employee dies.

All correspondence and completion of forms with the beneficiary(s) will be handled directly from the Benefits Office.

Life Insurance benefits will be paid directly from Reliance Standard Life Insurance Company to the beneficiary(s).
