Benefits

The Diocese of Raleigh dental plan is administered by the Christian Brothers Employee Benefit Trust. Coverage for employees and enrolled dependents is the same. Employees and covered dependents may go to any dentist they choose. The plan does not provide a network of dentists.

Coverage is provided within three different categories of procedures:

• Diagnostic/Preventative Procedures – benefit is paid at 80% of usual & customary charges
• Basic Procedures – benefit is paid at 80% of usual & customary charges
• Major Procedures – benefit is paid at 50% of usual & customary charges

There is no annual deductible requirement under the Dental Plan. The maximum benefit payable for all Dental Covered Charges incurred by a covered person during a calendar year is $1,000.

A claim form is not necessary for basic and preventative procedures, unless the procedure exceeds $200. Treatments that will exceed $200 must be filed with CBEBT before treatment begins. If needed, a copy of the CBEBT dental claim form (PDF) can be downloaded here.
